Please use the schedule below to register for 20-minute meetings with Lisa Peklak, RN from Attentive Health.
These meetings will be held by phone.
At your meeting, you can review the results of your Personal Health Survey, get support from Lisa, and answers to any questions you may have. This meeting with a Health Coach, along with the completion of the Passport to Health is due by June 1, 2020 to receive your employer's $10 per pay wellness credit beginning August 1, 2020
