Nutrikids Premium End of Year / Start of Year Schedule

1. If you would like us to complete both EOY and SOY you will need to book a separate appointment for each. EOY must be scheduled prior to SOY.
2. Choose  a Date and Time, click on an empty White date and time slot to create a new appointment. Blue time slots have been booked already
3. In the Description field provide ONLY your Districts PO # for billing purposes
4. Fill in the required information (Please verify that the email address is correct as this is where the appointment confirmation email will go)
5. Please select a Service Choice of either End of Year or Start of Year
6. Click the "Create Reservation" button
